Robotic Surgery vs Laparoscopic: What's the Difference?

Prof Uzma Mahmood

Both robotic-assisted surgery and traditional laparoscopic (keyhole) surgery are minimally invasive techniques used in gynaecological procedures. While they share many advantages over open surgery, there are important differences that may influence which approach is most suitable for your case.

What Is Laparoscopic Surgery?

Laparoscopic surgery, also known as keyhole surgery, involves making small incisions in the abdomen through which a camera and surgical instruments are inserted. The surgeon operates by viewing a 2D screen while manipulating instruments from outside the body. It has been the standard minimally invasive approach for decades and is highly effective for many gynaecological conditions.

What Is Robotic-Assisted Surgery?

Robotic-assisted surgery uses the Da Vinci surgical system, where the surgeon sits at a console and controls robotic instruments inserted through small incisions. The system provides enhanced 3D high-definition visualisation, 360-degree instrument articulation, and tremor filtration for greater precision.

Key Differences

Visualisation: Robotic surgery provides magnified 3D vision compared to the 2D view in standard laparoscopy, allowing the surgeon to see fine anatomical details more clearly.

Instrument movement: Robotic instruments have a greater range of motion than standard laparoscopic tools, enabling more precise movements in confined spaces such as the pelvis.

Precision in complex cases: For deep infiltrating endometriosis involving the bowel, bladder, or ureters, the enhanced precision of robotic surgery can be particularly beneficial.

Recovery: Both approaches offer faster recovery than open surgery, with most patients going home the same day or the following day. Recovery times are generally comparable between the two techniques.

Suitability: Standard laparoscopy is excellent for straightforward procedures. Robotic surgery is often preferred for complex cases requiring intricate dissection, such as advanced endometriosis, large fibroids, or cases with significant scar tissue from previous surgeries.

Which Approach Is Right for You?

The choice between robotic and laparoscopic surgery depends on the nature and complexity of your condition, your surgical history, and your overall treatment goals. Prof Mahmood is trained in both techniques and will recommend the approach that offers the best outcomes for your individual case.

Not all patients require robotic surgery, and Prof Mahmood will provide a clear and honest recommendation based on your specific situation.
